We at Body Limits Ltd (“We”) would like to promise to look after your personal data. We collect the minimum information we need to be able to do the job we do.
We are the data controller however we use a third party system to be able to take bookings and store the information. We are responsible for taking all information from you and making sure it is correct by checking when you arrive in the clinic.
2. What types of personal data do we use?
We have your:
• Date of birth
• Email address
• Contact numbers
• Physical or Mental Medical health history that you disclose to us
• Treatment you have received and how/what payment has been made
• Reports from GP or hospital that you have given consent to be sent to us or brought from your own records
We do not store any card details at Body Limits, the receipt will contain your card number in full and expiry date. No CVV numbers or names will appear on the card receipt.
We do not collect sensitive data such as racial or ethnic origin, political opinion, religious beliefs.
3. Where will we collect personal data from?
Data is collected from talking to you on the phone, or through our online system, or email and social media messaging. This data is what you choose to tell us and we store this accordingly.
4. Who do we share your data with?
We share your data only with therapists within the clinic if you are seeing multiple therapists. We will share your data, with consent first, with other medical professions such as GP and imagery consultants when you have requested reports. If you are involved in an accident then your insurance company may also request medical details, this will be with consent from them first.
5. How do we use your data?
We use your data for consultation purposes, we may use your date of birth to help with that diagnosis.
We also send you messages to either your email or mobile number to inform you of your appointments.
As requested from yourself we may need to use your address for the purpose of a receipt after payment has been made.
Our computers are logged off once therapists leave the rooms or put in to the private setting if the therapist leaves the room leaving the computer and patient together.
We may need to send you exercises via your email address though our third party program called Rehab my patient. This only stores your email address and no other details.
6. How long do we keep your data for?
We do not hold our information for longer than necessary.
We keep your data for 7 years post last treatment – an adult is 18yrs or over.
Up to the age of 18.
